Introduction: The focus on social capital in recent years has been based on the effect of this capital onproducing and increasing the human capital and also improving the level of general health in the society.The social capital is discussed as a potential in successful social welfare programs and general health; andfor the same reason, a specific place has been assigned to investigate and codify the social, physical andmental health indices. The purpose of this study was to identify the relationship between social capital andmental health among the social sciences students in University of Isfahan, Iran.Methods: The investigation was conducted using descriptive and correlation studies. Initially, theconcepts of social capital and mental health were identified using script studies and then the data werecollected and expressed. The relationship between students’ social capital and mental health wereanalyzed using SPSS software.Results: The findings showed an inverse statistically significant correlation between confidence,belongingness, affection, and trustworthiness with mental health. Based on the findings of the study, therewas a relationship between students’ social capital and mental health.Conclusion: One of the significant indices was the low level of communion, confidence, belongingness,affection and trustworthiness among students which is an indication of lower social capital among them

Introduction: Recently, the accreditation program has been used increasingly by an emphasis onpatient safety. In Iran, efforts have been made to establish the accreditation program. In 2011, itreplaced previous standards of traditional evaluation system. As the impact of accreditation planneed internal self assessment against minimal standards, this study aimed to survey the readinessof selected hospitals in Isfahan based on the functional model of Joint Commission onAccreditation.Methods: This cross-sectional descriptive study was conducted on four hospitals in Isfahan, Iran.Data was collected using 13 checklists based on the functional model of Joint CommissionInternational Accreditation. The checklists were verified by the Iranian Ministry of Health. Datawas collected via observations and interviews with hospital representatives in qualitymanagement. All statistical analyses were performed using SPSS.Results: The readiness of the selected hospitals in Isfahan was 64% regarding patient-basedstandards and 66% for facility-based standards. In general, the studied hospitals had adequatereadiness for implementation of accreditation standards.Conclusion: As the study population has ISO 9000 certification, it is possible to replace oldmethods of process, outcome, and structure evaluation with new standards. Accordingly, patientrights have to be considered by health care employers in Isfahan.

Introduction: Today, many organizations attempt to fully develop in gradual evolution of their processes. PCMM is one of the models which focus on improving organizational human capabilities. This model is applied to continually increase people's ability to attract, develop, motivate, organize and retain the skills. In this study, PCMM was used to investigate organizational maturity needs in medical record departments of Isfahan public hospitals and to determine strengths, weaknesses, skills and capabilities of their staff members. Methods: This was an applied, cross-sectional study in which data was collected by questionnaires. The questionnaires were given to the staff of Isfahan public hospitals to investigate PCMM model needs at levels 1 and 2. The questionnaire has been extracted from the PCMM model and its validity and reliability have been approved by Cronbach's alpha (α = 0.96). The collected data was analyzed by SPSS using descriptive statistics. Results: Our findings showed that the mean score of skills and capabilities of medical record staff in governmental hospitals was 35 (out of 56). Medical record staff of hospital number 10 had the maximum maturity in level 2 of PCMM model (mean score = 55). Hospitals number 1 and 4 had minimum maturity in level 2 of PCMM model (mean score = 24). There was no significant relation between organizational maturity and the characteristics of medical record staff. Conclusion: PCMM model application would lead the staff and managers to pay increased attention to identifying the weaknesses of current activities and practices to improve and develop the onging processes.

Introduction: Hospitals are responsible to respond to legitimate demands for health information release while protecting the confidentiality of patient health records. This study investigated and compared laws and policies of health information disclosure for legal authorities in Iran and some selected countries. Methods: This descriptive-comparative study included health information disclosure laws and policies in selected countries such as the United States, Australia, England, Malaysia and Iran. Data were gathered from systematic internet search, library resources and communication with health information professionals. Data analysis was done using comparative tables and qualitative method. Results: The results showed that in all the five studied countries, there are specific written policies and procedures for health information disclosure for legal authorities. It was found that, without a judicial warrant, a patient consent is needed for medical record disclosure for courts, representatives of judicial authorities and law enforcement agencies. Conclusion: In all of the selected countries there are ordained limitations for health information release to protect patient rights. Keywords: Disclosure; Legislation; Medical Records.

Introduction: Quantitative analysis of medical records is effectiveness factors in hospital policies, assessment of treatment team performance, and improve executive activities in hospital and they are the principle tool for medical audit. Despite the importance of evaluation and high cost and time, it was not intentioned enough. This paper review the result of Quantitative analysis of medical records of patients admitted in the Gharazi hospital of Isfahan, Iran. Methods: In this descriptive and cross-sectional study, the medical records of all patients admitted in the hospital from the first April 2008 until the end of March were evaluated. Sampling method was census and 26284 number of cases were studied Data collection tool was the checklist that was extracted from medical record text. Researcher referred to the medical record department and data from hospital records were extracted and compiled the checklist and analyzed by SPSS. Results: The most frequency of failure of patients file were related to Ear, Nose and Throat (ENT) specialists (38%) and Maxillofacial Surgery department professionals had not any defect in their patients file record. Conclusion: Quantitative analysis of medical records Based on the desired topics of documentation showed that the most failure of patient records were related to documentation of date entry by documentary makers (91%) and the lowest deficit was related to data entry (1.1%). Few percent of the cases (1.2%) were not assessed in medical record. Considering the results of this study, the Gharazi hospital of Isfahan, Iran is planning to control the case documentation and medical records evaluation by applying appropriate management tools. Introduction: Disaster occurs almost daily in the world and increases the issue of it because of high volume of population, industrialization and acts of terrorism, my country is one of the most unexpected event areas of the world. Health care systems encounter special challenges in disaster management, that they include triage and treatment a large number victims and also victims' information management. To ensure correct way of documentation of victims as introduction information management and effective triage and treatment, useful medical record are necessary, because medical record usually used for patients are too complicated and ineffective to use in case of emergency.So the objective of this research is design the disaster-victim medical record to document and triage easily for victims in disaster.Methods: This research is an applied study and has been performed as descriptive study. Source of information is libraries and accepted articles in indexed journals. During survey of corresponding organizations about disaster in Isfahan and absence a medical record for documentation of victims, researchers identified the organizations of disaster in the world and then gathered necessary data elements for medical record in disaster. Then necessary parameters for the medical record extracted from American and European models.Results: According to situation and standards of Iran, a proper and final pattern designed with assisting management and medical emergencies center in Isfahan province.Conclusion: Medical record in disaster relief operations must be simple and useful; medical record used in hospitals isn't effective. The proposed model is simple and proper form for documentation of victims and easy reference for accessing clinical, administrative and statistics information in disaster.Keywords: Disasters; Triage; Medical Record; Safety Management; Information Management.

Introduction: Formation of professional associations is one point of inflection in the health care industry. Health Information Management Association in developed countries originally initiated its activities in the profession of medical records as Medical Records Librarians Association and later changed to Medical Record Association. This study aimed to compare structure, function and activities of Health Information Management Association in selected countries and Iran.Methods: This was a descriptive comparative study in cross-sectional method. The research sample included Health Information Management Association in Australia (HIMAA), United States of America (AHIMA) and Canada (CHIMA) as pioneer countries in initiation of Health Information Management Association and also Medical Record Association of Iran (IMRA). Related data about structural and functional entities was collected through the library investigation, internet and communication with research sample associations via email according to collection forms.Results: The structural and functional elements and entities of the Health Information Management Associations in the selected countries are mostly similar, although they had some differences. Canadian Health Information Management Association Board of Director’s is communal with Canadian College of Health Information Management. Type of association memberships in Australia is different with other associations. Vision and values of HIMAA and IMRA have not been identified and written. In comparison with the other associations, AHIMA has various and multiple activities in provide services to its members. Continuing education and certification in various scope is one of major the activities of AHIMA.Conclusion: commensurate with done Changes in the Health Information Management Profession, the Association of this Profession in countries of Australia, United States and Canada has experienced a lot of change in the structure and activities of their associations accordingly. Position of inspector only existed in Iran's Medical Record Association. In comparison with other association, Iran Medical Record Association with fifteen years of history had not had a lot of systematic and integrated activities. Introduction: Medical Records Department (MRD) is an important source for evaluating and planning of care services. The main goal of this research was performance evaluation of medical records department with Balanced Scorecard (BSC) approach in a hospital of Isfahan, Iran. Methods: This research was a descriptive, cross sectional in which data were collected by questionnaires, observation and forms. The population was medical records department of a hospital in Isfahan, Iran. To analyze data first of all objectives of medical records department according to mission and perspectives was setting. Then objectives were measured. Subsequently, findings from the performance will compare with expected score. If necessary, reform for the programs, activities, and plans in order to achieve the final target will be done. Results: The medical records department of this hospital was successful in providence of customer suggestion value (satisfaction). In customer perspective was customer satisfaction of admission 82%, statistics satisfaction 83%. Conclusion: Researcher proposes that it had better improve quality and period length of services, for achiving the mission of medical records department.Keywords: Performance Assessment; Medical Records Department Hospital; Hospitals.

Introduction: The information recorded in medical sheets can help us facilitate the treatment process, recognize the quality of performance of the medical staff, plan health care programs and make accurate and appropriate decisions. This study intends to compare the quality of recording information in admission and summary sheets of three hospitals in Isfahan in the first six months of 1381. Materials and methods: This is retrospective study in which the information recorded in the admission and summary forms of 571 in-patients in three hospital in Isfahan were descriptively examined in seven checklists. Results: The private hospital enjoyed the highest frequency of recording information in admission and summary sheets. The frequency of recording information in educational and private hospitals was similar. However, the frequency of recorded information in the two hospitals indicated a meaningful difference that in the private hospital. Discussion: The findings reveal that admission and summary sheets are not completely filled in and the responsible authorities must pay due attention to the process of recording medical information in the hospitals.
